Special Meal Accommodations
Placentia-Yorba Linda Unified school District provides modified menus for students who have a disability, medical condition, or special dietary need and are unable to consume a regular breakfast or lunch. Our policy is in accordance with a federal mandate to include all students in school nutrition programs.
Meal Substitutions for Students with Disabilities or a Medical Condition
USDA Regulation 7 CFR Part 15b requires substitutions or modifications in school meals for children whose disabilities restrict their diets. A child with a disability must be provided substitutions in foods when that need is supported by a signed statement from a recognized medical authority (licensed physician, physician assistant, or nurse practitioner). Each special dietary request must be supported by a statement explaining the requested food substitution and must be signed by a state licensed healthcare professional (licensed physician, physician assistant, or nurse practitioner). Click below for medical statement form. The medical statement form must be completed in its entirety. Medical statements completed by parents or guardians will not be accepted. Please return all completed forms to your school front office or school nurse.
